• NO middle men deciding your benefits

Prices

Child: $249/year (ages 12 and under)

Adult: $299/year (ages 13 and above with no periodontal history/disease)

Restrictions

• Your In-House Membership Plan is active for one year from date of purchase and renews every 12 months from purchase.
• Your In-House Membership Plan yearly fee is non-refundable.
• You will be charged $50 for missed appointments or “no shows,” or you may choose to forfeit one exam and cleaning in lieu of the $50 charge. No charge or forfeiture for appointments cancelled or rescheduled 24 hours in advance.
• Your In-House Membership Plans may be cancelled without a refund for frequent cancellations, reschedules, or “no shows.”
• In-house plans cannot be combined with any other special offers, discounts, insurance, or third-party reimbursement.
• Your In-House Membership Plan is not valid at other dental offices or referred specialists.
• Your In-House Membership Plan is not insurance. It is a discount through Nuckols Family Dentistry.
